Opioids have held a prominent role in the management of moderate to severe perioperative pain in children despite rising concerns over their use and risk of opioid-induced ventilatory impairment (OIVI).1 As anaesthetists, we must consider multiple pharmacological, social, psychological, legal and environmental factors when prescribing and administering an opioid to paediatric patients.2
